A copy of your most recent Canada Revenue Agency Notice of Assessment (NOA) is required with this application

REFERENCE – A reference may be provided for use in extenuating circumstances and upon approval from the PROKIDS Administrator. The reference must be familiar with your personal and financial situation, who can verify that you require financial assistance from PRO Kids. This person should be an adult (not a relative or friend) who knows the family. (Example: Social Worker, Community Services, Clergy, Family Resource Center, Group Leader, Teacher)

Parental Consent -I authorize the above reference to release relevant personal information as required by PROKids. I further authorize PROKids to collect this information for administration purposes including release to program providers. This application is confidential and used solely for the purposes of PROKids records.

To verify the cut-offs, we require your most recent tax year Notice of Assessment (one for each parent/guardian in the household), or an endorsement from the Department of Community Services, or an endorsement from Indigenous Social Assistance. Your total family income (line 15000) must be less than the cut-offs listed for your family size:

*To calculate family size count the number of parent or parents in the household plus the number of children 18 years or younger.

Example:
2 person family = 1 parent + 1 child.
3 person family = 1 parent + 2 children OR 2 parents + 1 child
